Endocrinologist
You manage the body's hormone systems – and diabetes and thyroid disease are rising rapidly across Africa.
What this life looks like
Hormones control almost everything in the human body – growth, metabolism, reproduction, stress responses, blood sugar, sleep, and mood. As an Endocrinologist you specialise in diagnosing and treating conditions of the glands that produce these hormones: the thyroid, pancreas, adrenal glands, pituitary, and reproductive organs. The most common endocrine conditions – diabetes, thyroid disease, polycystic ovarian syndrome, and adrenal disorders – affect tens of millions of Africans, mostly undetected and untreated. Diabetes alone affects an estimated 24 million adults in Africa and the number is projected to more than double by 2045, driven by urbanisation, dietary change, and physical inactivity. Yet Africa has one of the lowest densities of endocrinologists in the world, meaning most people with these conditions are managed sub-optimally without the specialist knowledge to prevent the devastating complications of poorly controlled diabetes – blindness, kidney failure, amputations, and heart disease. The Endocrinologist who builds a practice in Africa is addressing a silent epidemic.
What she actually does
An Endocrinologist diagnoses and manages diabetes mellitus including both Type 1 and Type 2, thyroid disorders, adrenal disorders, pituitary tumours and dysfunction, metabolic bone disease, polycystic ovarian syndrome, and other hormonal conditions. They prescribe and monitor medications including insulin, manage the complications of endocrine disease, educate patients on long-term self-management, and consult for other specialties on patients whose endocrine disorders complicate their other conditions.

Why it matters in Africa
The diabetes epidemic in Africa is one of the most significant public health challenges of the coming decades. Complications of poorly controlled diabetes – diabetic foot leading to amputation, diabetic nephropathy leading to dialysis, diabetic retinopathy leading to blindness – are devastating individuals, families, and health systems. Thyroid disease, particularly iodine deficiency causing goitre and hypothyroidism, remains a significant problem in parts of Africa where iodised salt programmes are incomplete.
Your pathway in
Biology, Chemistry, and Mathematics at secondary school. Then MBBS or MBChB over five to six years, followed by housemanship. Internal medicine residency then subspecialty fellowship in endocrinology through the West African College of Physicians or equivalent body follows. The full pathway takes approximately 12 to 14 years.
Skills you will need
Technical: Diabetes management including insulin therapy, thyroid function interpretation, hormonal investigation and interpretation, management of endocrine emergencies including diabetic ketoacidosis, and ultrasound for thyroid and adrenal assessment. Human: Long-term relationship management because endocrine conditions are usually chronic, patient education and motivation for lifestyle change, the ability to explain complex hormonal concepts in accessible language, and sensitivity around conditions like PCOS that affect women's fertility and body image. Digital tools: Continuous glucose monitoring systems, insulin pump technology, digital diabetes management platforms, and telemedicine for remote endocrine consultation.
A woman in this field
Dr. Afokoghene Isiavwe
Nigeria
Nigerian endocrinologist and diabetologist with over 26 years of experience, and founding partner of Rainbow Specialist Medical Centre in Lagos. She coordinated the first Podiatry and Diabetes Foot Care Conference in Nigeria and created the Diabetes Podiatry Initiative, a training programme on diabetes foot screening designed for primary care doctors, paramedics and nurses that has since expanded across Lagos and the rest of the country. As Coordinator of the Diabetes Podiatry Initiative in Nigeria, she has initiated several local and national campaigns to raise awareness of diabetes and its complications. She is a founding member of Diabetes Foot Nigeria and an active advocate for multidisciplinary diabetes care across West Africa.
Take your first step
Biology and Chemistry are your core subjects at school – endocrinology is deeply rooted in biochemistry, the study of chemical processes in the body. If you have family members with diabetes or thyroid conditions, you already have a personal connection to this specialty. The International Diabetes Federation Africa region website has accessible information about the scale of the diabetes epidemic in Africa. Start building your understanding of how insulin works and why blood sugar control matters – these concepts will accompany you throughout your entire career.
